Parent/Guardian Acknowledgment
By entering my name below, I acknowledge that I understand that participation in Tri-Cities Prep Sports Camp involves physical activity and the risk of injury. I voluntarily allow my child to participate and assume the risks associated with camp activities. I agree to hold harmless Tri-Cities Prep, its employees, coaches, and volunteers from claims arising out of my child's participation, except in cases of gross negligence or willful misconduct. I authorize emergency medical treatment if necessary and understand that I am responsible for any resulting medical expenses.
